General website forms are for non-urgent requests only. Do not submit medication lists, photos, urgent symptoms, diagnosis details, or treatment instructions through a general contact form. The dedicated Wellness Consent form separately collects the participant information, date of birth, waiver acceptance, and typed electronic signature needed to document that consent.
When you submit the Wellness Consent form, True Bliss Medical receives an emailed copy containing your name, date of birth, typed signature, acceptance of the waiver, submission time, waiver version, and a technical request identifier. This information is used to document event consent and is available only to authorized recipients.
If you choose text follow-up and check the SMS consent box, True Bliss Medical may text you about your request. Message and data rates may apply. Reply STOP to opt out. Consent is not required to receive care or buy services.
The clinic uses submitted information to respond to inquiries, schedule consultations, manage front desk follow-up, and maintain internal CRM records for staff accountability.
The website may record technical details such as pages visited, referral source, approximate location from Cloudflare, browser information, Cloudflare request IDs, and IP address for site security, abuse prevention, troubleshooting, and aggregate analytics.
CRM access is limited to authorized staff accounts. Staff activity, lead updates, consent capture, and communication events are recorded in the CRM activity history.
